About the Role:

This is an exciting opportunity to provide person-centered support for people with mental health issues, focusing on achieving meaningful health, social, and economic outcomes.

Key Responsibilities:
  • Engage participants and their families in program activities, developing trusting relationships.
  • Conduct regular face-to-face visits to connect with clients, monitor progress, and offer support.
  • Encourage linkages into various activities to achieve goals, including connections to other support services.
  • Develop individualized recovery plans with each participant, incorporating specific goals and focusing on skill and knowledge development.
  • Provide direct practical support to help clients attain skills required to meet identified goals.

Regularly review service delivery to ensure alignment with individual needs.

Requirements and Qualifications:
  • Relevant qualifications or experience in support roles, typically a Certificate IV qualification or above.
  • Valid driver's license and ability to travel for the role.
  • Working with Children Check and NDIS Workers Screening Check.
  • Right to Work within Australia.
  • Satisfactory National Police Records Check.
  • NDIS Worker Orientation Module Certificate.
