Care Coordinator Role

We are seeking a compassionate and skilled Care Coordinator to join our team. As a key member of our care delivery team, you will play a vital role in ensuring the high-quality support and services are provided to our clients.

Your primary responsibility will be to provide expert guidance and support to clients, their families, and caregivers. This includes conducting thorough assessments, developing personalized care plans, and coordinating the delivery of these plans with our care team.

You will also be responsible for managing client enquiries, responding to phone calls and emails, and addressing any day-to-day issues that may arise. In addition, you will work closely with our Rostering team to ensure efficient service delivery and maintain accurate and compliant documentation.

This is an exciting opportunity for an experienced and passionate Care Coordinator who is committed to delivering exceptional care and support to our clients.

Key responsibilities include:

  • Providing expert guidance and support to clients, their families, and caregivers.
  • Conducting thorough assessments and developing personalized care plans.
  • Coordinating the delivery of care plans with our care team.
  • Managing client enquiries and responding to phone calls and emails.
  • Addressing day-to-day issues that may arise.

Required skills and qualifications:

  • CURRENT AHPRA REGISTRATION (RN) SPECIALIZED IN AGEING OR DISABILITY & COMMUNITY
  • AUSTRALIAN NATIONAL POLICE CHECK
  • WORKING WITH CHILDREN CHECK
  • FIRST AID & CPR CERTIFICATE
  • DRIVER'S LICENSE
  • EXCELLENT VERBAL AND WRITTEN COMMUNICATION SKILLS
  • PROFESSIONAL INDEMNITY INSURANCE (RN)
  • SOLID KNOWLEDGE IN THE DISABILITY AND/OR AGED CARE SECTOR(S)
  • CLINICAL SKILL IN WOUND CARE
  • TIME MANAGEMENT AND ORGANIZATIONAL SKILLS
  • RELATIONSHIP DEVELOPMENT SKILLS
  • INTERMEDIATE TO ADVANCED MICROSOFT OFFICE EXPERIENCE
