The role of a Care Coordinator is highly visible and plays a vital role in meeting our residents' daily needs and delivering person-centered care to residents and families.
Our Care Coordinators are responsible for the total care of the resident, including physical, emotional wellbeing and safety. They are given the autonomy and space they need to make decisions and enjoy the support of the personal care, lifestyle, and hospitality teams.
In addition to this, they act as role models by adhering to safe practice measures in medication management, providing leadership to others and by doing so promote a harmonious team environment aligned with our values.

Your Key Responsibilities- To provide high-quality care to our residents, ensuring their physical, emotional, and social needs are met.
- To work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs of each resident.
- To maintain accurate and up-to-date records of resident information, including medical history, medications, and progress notes.
- To communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
- To participate in staff meetings and training sessions to stay updated on best practices and company policies.
- A Bachelor of Nursing degree or equivalent qualification.
- AHPRA Registration with no restriction.
- Strong communication and interpersonal skills, with the ability to build rapport with residents, families, and colleagues.
- An empathetic and patient approach, with the ability to work in a fast-paced environment.
- Ability to think critically and solve problems effectively.
- Basic computer skills, including proficiency in electronic medical records systems.
- A supportive and collaborative work environment.
