Key Responsibilities:
  • Provide case management support to individuals who are homeless or at risk of homelessness.
  • Develop personalized plans to meet the needs of each participant.
  • Offer practical and personal assistance, including crisis budgeting, information, advice, and community referrals.
  • Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as required.
  • Advocate for participants to access housing and specialist services.
Mandatory Qualifications:
  • Certificate IV in Community Services or equivalent qualification.
