As a Clinical Care Coordinator, you will play a vital role in delivering comprehensive care to children and youth. Your primary focus will be on providing high-quality evidence-based clinical care as part of a multidisciplinary team.
The role involves addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention. You will promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
Key responsibilities include:
- Contributing to the development and implementation of care plans that cater to the unique needs of each individual or family.
- Collaborating with colleagues to ensure seamless communication and coordination of services.
- Engaging with clients, families, and carers to understand their concerns and provide empathetic support.
- Participating in regular team meetings and professional development activities to stay up-to-date with best practices.
To be successful in this role, you will need to possess:
- A relevant qualification in occupational therapy, mental health nursing, psychology, or social work.
- Current registration with AHPRA (Australian Health Practitioner Regulation Agency) or equivalent.
- Experience working with children and youth in a mental health setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
