We are seeking a skilled Medical Care Coordinator to join our team at WellLife Network. As a Medical Care Coordinator, you will play a critical role in ensuring the health and well-being of our residents.
Key Responsibilities- Conduct thorough assessments of residents' medical needs and develop individualized care plans to address their unique requirements.
- Collaborate with interdisciplinary teams to coordinate medical services, including medication management, wound care, and other specialized treatments.
- Provide education and training to residents and staff on health and hygiene practices, as well as disease prevention and management.
- Participate in quality improvement initiatives to ensure compliance with regulatory standards and best practices in healthcare.
- Develop and maintain relationships with healthcare providers, including physicians, hospitals, and pharmacies, to ensure seamless coordination of care.
- Administer TB tests and other medical screenings as required.
- Ensure accurate and timely documentation of medical records and reports.
- Perform other duties as assigned by the supervisor to support the delivery of high-quality medical care.
To be successful in this role, you will need:
- A current NYS license and registration by the NYS Department of Education.
- Specialized training or one year of experience in treating and caring for individuals with developmental disabilities.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, staff, and healthcare providers.
- A strong commitment to providing high-quality, patient-centered care.
