Job Summary

We are seeking a skilled Medical Care Coordinator to join our team at WellLife Network. As a Medical Care Coordinator, you will play a critical role in ensuring the health and well-being of our residents.

Key Responsibilities
  1. Conduct thorough assessments of residents' medical needs and develop individualized care plans to address their unique requirements.
  2. Collaborate with interdisciplinary teams to coordinate medical services, including medication management, wound care, and other specialized treatments.
  3. Provide education and training to residents and staff on health and hygiene practices, as well as disease prevention and management.
  4. Participate in quality improvement initiatives to ensure compliance with regulatory standards and best practices in healthcare.
  5. Develop and maintain relationships with healthcare providers, including physicians, hospitals, and pharmacies, to ensure seamless coordination of care.
  6. Administer TB tests and other medical screenings as required.
  7. Ensure accurate and timely documentation of medical records and reports.
  8. Perform other duties as assigned by the supervisor to support the delivery of high-quality medical care.
Requirements

To be successful in this role, you will need:

  • A current NYS license and registration by the NYS Department of Education.
  • Specialized training or one year of experience in treating and caring for individuals with developmental disabilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, staff, and healthcare providers.
  • A strong commitment to providing high-quality, patient-centered care.
