**Thriving in this role requires**:

- **Experience in home care package coordination**: You'll be experienced at navigating the intricacies of home care, including managing budgets and ensuring efficient and effective service delivery and
- **A passion for client care**: Building genuine relationships with your clients and their families will be paramount. Your expertise will empower them to live independently with the support they deserve.
- In addition to all of the the above, you will also need:_
- Demonstrated working knowledge of the Aged Care Quality Standards and compliance requirements for Home Care Packages
- Proven ability to work effectively with colleagues to achieve the best outcomes for our clients
- Good computer skills including with care databases, word, excel, outlook
- National Police Check
- Current Australian Drivers Licence
- Current First Aid and CPR
- Right to Work in Australia permanently with no restrictions

We are seeking a self-motived and dynamic Care Coordinator with Case Management experience of home care (CHSP) clients living in the community. You will enjoy working as part of a dynamic multi-disciplinary team to ensure high quality services are provided whilst building strong relationships with consumers and their families.

Within this role you willbe required to do the assessment, coordination, care planning, monitoring to ensure client outcomes are achieved, work closely with direct care workers, providing mentoring and guidance where needed.

To be successful in this role you will have:

- Genuinely passionate about caring for the elderly
- Be Self-motivated with a "can do" and flexible attitude
- High level communication, analytical, problem solving and organisational skills
- Knowledge and ability to work with people from a wide range of backgrounds and cultures
- A keenness to network and promote our services
- Demonstrate ability to use computer systems including MS Word and Excel
- Drivers Licence

You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain). An ability to speak another language will be highly regarded.
