Case Worker Opportunity

We are seeking a proactive Case Worker to provide efficient day-to-day service delivery through education, interventions, group work, assessment, referral and case work within a recovery-orientated and person-led service environment.

This is a permanent, full-time position. Salary and conditions are in accordance with SCHADS Level 4.

Your Key Responsibilities:

  • Develop and monitor individual treatment plans and maintain case management documentation, records, reports and data collection, utilising established case planning procedures.
  • Assess client and program risk and mitigate risk in accordance with our policy and procedures.
  • Demonstrate commitment to an environment that values continuous improvement and innovation in the delivery of high-quality services.
  • Provide reports (oral and/or written) to Team leader as requested.
  • Acquire and maintain a good working knowledge of referral and support options for clients.
  • Work collaboratively and professionally with other services.
  • Participate in sector events and training.

Your Requirements:

  • A valid NSW Driver Licence.
  • First Aid certificate.
  • Minimum of Diploma level qualification in a relevant discipline (AOD, Community Services, Case Management).
  • Completed 4 AOD Skill Set Units (or willing to undertake/complete during employment).
  • Demonstrated ability to deal with unpredictable situations and behaviours utilising de-escalation techniques.
  • The ability to critically reflect on professional practice and attitudes.
  • The ability to develop and tailor programs to meet client's needs.
  • The knowledge of AOD, mental health, community services and health sectors.
  • Demonstrated experience and ability to work with those impacted by substance abuse, gambling and mental health conditions.
  • Experience working with people from Aboriginal and Torres Strait Islander background.
  • Experience working with people from culturally and linguistically diverse backgrounds and religions.
  • Experience working in collaboration with other organisations.
