About the Role:
  • Perform assessments based on clinical expertise, including ADL's, functional capacity evaluations, worksite, and ergonomic assessments;
  • Collaborate with stakeholders including clients, medical specialists, insurers, and other treatment providers to achieve a common goal
  • Work with clients to identify barriers and strategies to overcome them;
  • Maintain case manager records;
  • Develop, implement, and monitor rehabilitation plans for individuals across the region.
Requirements:
  • Current registration with relevant industry body (e.g., AHPRA, ESSA)
  • Tertiary qualifications in Occupational Therapy, Physiotherapy, Exercise Physiology, Nursing, Social Work, Rehabilitation Counselling, or Osteopathy
  • Clinical reasoning
  • Ability to engage across multidisciplinary teams to support goal achievement
  • Passion to help people recover
  • Ideal experience working with veterans or similar cohorts
  • Occupational rehabilitation experience is preferred

Key Responsibilities:
  • Assessment and Planning: Perform various assessments to determine individual needs and develop tailored rehabilitation plans;
  • Collaboration and Communication: Work closely with stakeholders to achieve common goals and maintain open communication throughout the process;
  • Client-Centred Approach: Focus on client-centred care, identifying barriers, and developing strategies to overcome them;
  • Record Keeping: Maintain accurate and detailed case manager records;
  • Program Development: Develop, implement, and monitor rehabilitation programs to support individuals in returning to work and daily life.
Skills and Qualifications:
  • Registration and Certification: Current registration with relevant industry bodies (e.g., AHPRA, ESSA);
  • Education and Training: Tertiary qualifications in Occupational Therapy, Physiotherapy, Exercise Physiology, Nursing, Social Work, Rehabilitation Counselling, or Osteopathy;
  • Clinical Reasoning: Strong clinical reasoning skills to inform assessment and planning decisions;
  • Teamwork and Communication: Ability to engage across multidisciplinary teams and communicate effectively with stakeholders;
  • Rehabilitation Experience: Ideal experience working with veterans or similar cohorts, with a preference for occupational rehabilitation experience.
