Job Description

We are seeking a dedicated and compassionate care advisor to join our team in Bundaberg. As a key member of our community-based care team, you will provide high-quality support to our clients, ensuring their health and wellbeing needs are met.

Key Responsibilities:

  • Conduct in-home assessments of participants
  • Provide ongoing case management, supporting both clinical and holistic needs
  • Monitor participant wellbeing and update care/case/health plans accordingly
  • Document and track participant goals, progress, and outcomes using person-centred, wellness, and reablement strategies
  • Liaise with regulatory bodies
  • Provide support, training, and guidance to colleagues
  • Assist with service scheduling and respond to urgent care placements or changes
  • Advanced computer and administration skills required
  • Excellent multitasking, prioritisation, and problem-solving abilities
  • Proven ability to work independently and as part of a team
  • Previous experience working to KPIs and tight deadlines
  • Current CPR/First Aid Certificate
  • Valid Australian Driver's Licence

Desirable Qualifications:

  • Experience in home care packages, community care, or aged care/health sector
  • Understanding of Home Care Package Program guidelines and compliance
