Job Title: Home Care Advisor Clinical Registered/Enrolled Nurse

We are seeking a skilled and compassionate home care advisor to join our team. As a clinical registered or enrolled nurse, you will be responsible for conducting in-home assessments of participants, providing ongoing case management, and monitoring their wellbeing.

Key Responsibilities:
  • Conduct in-home assessments of participants to understand their needs and develop personalized care plans.
  • Provide ongoing case management, supporting both clinical and holistic needs, and monitor participant wellbeing to ensure the best possible outcomes.
  • Document and track participant goals, progress, and outcomes using person-centred, wellness, and reablement strategies.
  • Manage participant budgets and ensure compliance with documentation and reporting standards.
  • Liaise with relevant regulatory bodies and provide support, training, and guidance to colleagues as needed.
Requirements:
  • Clinical experience in aged care or healthcare
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Advanced computer and administration skills
  • CPR/First Aid certification and a current Working with Children Check
