Job Title: Home Care Advisor Clinical Registered/ Enrolled Nurse in Bundaberg

About the Role

We are seeking a skilled and compassionate Clinical Registered/Enrolled Nurse to join our team in providing high-quality care to clients in their homes. As a member of our home care team, you will be responsible for conducting in-home assessments, providing ongoing case management, and monitoring participant wellbeing.

You will work closely with participants, families, and other healthcare professionals to develop and implement personalized care plans that meet the unique needs of each individual. Your expertise and experience will enable you to provide holistic care that addresses physical, emotional, and social needs.

Key Responsibilities:
  • Conduct thorough in-home assessments to identify participant needs and goals
  • Develop and implement personalized care plans that address clinical and holistic needs
  • Monitor participant wellbeing and update care plans as needed
  • Document participant progress and outcomes using person-centred strategies
  • Liaise with ACAT, My Aged Care, PRODA, and other regulatory bodies
Required Skills and Qualifications

To be successful in this role, you will require advanced computer and administration skills, excellent multitasking, prioritisation, and problem-solving abilities. You will also need to possess strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
