Job Title: Home Care Advisor Clinical Registered/Enrolled Nurse

We are seeking a skilled and compassionate clinical professional to join our team in Bundaberg. As a Home Care Advisor, you will be working in a community-based setting where you will have the opportunity to make a real difference in the lives of our clients.

Main Responsibilities:
  • Conduct in-home assessments of participants to identify their needs and develop care plans that meet their individual requirements.
  • Provide ongoing case management support to ensure the holistic needs of participants are met.
  • Monitor participant wellbeing and update care/case/health plans accordingly.
  • Document and track participant goals, progress, and outcomes using person-centred approaches.
  • Liaise with regulatory bodies, including ACAT and My Aged Care, to ensure compliance and optimal service delivery.
  • Provide training and guidance to support staff and colleagues to enhance overall service quality.
Requirements:
  • Advanced computer and administration skills, with proficiency in CRM software.
  • Excellent multitasking, prioritisation, and problem-solving abilities.
  • Proven ability to work independently and as part of a cohesive team.
  • Previous experience working to Key Performance Indicators (KPIs) and meeting tight deadlines.
  • Certification in CPR/First Aid and a valid Australian Driver's Licence are essential.
