Restorative Care Professional">

The role of Restorative Care Professional involves the coordination and delivery of restorative and rehabilitative care services. The primary goal is to ensure optimal health outcomes for clients while promoting independence and functional recovery.

This is a full-time, permanent position based at our Abbotsford office, with hybrid working arrangements available.

  • Community engagement and stakeholder interaction

  • Quality care processes and risk management
About You

To be successful in this role, you will have:

  • Current registration as a Registered Nurse or Allied Health professional
  • Specialist certifications or qualifications are highly regarded
  • Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
  • Strong understanding of reablement, functional rehabilitation, and person-centred care models
  • Proficiency in risk management, compliance, and quality assurance processes
  • High-level communication and problem-solving abilities
  • Competence in using clinical documentation and care management systems
  • Exceptional customer service and interpersonal skills
  • Proven experience working in a fast-moving environment, managing a high workload, and providing high-quality advice and support
  • A sound understanding of client needs and experiences
  • Competence in building productive networks and relationships
  • An ability to take initiative, solve problems, and make decisions
  • Experience working towards operational key performance indicators (KPIs) and achieving business development targets
  • Experience managing client budgets and being responsive to time-sensitive tasks
  • Driver's license and own car
