About the role…
The Restorative Care Partner coordinates and delivers restorative and rehabilitative care services according to each client's support plan. The role focuses on achieving optimal health outcomes, promoting independence, and ensuring culturally responsive, holistic, and inclusive care.
This full-time, permanent position is based at our Abbotsford, Vic office, with hybrid working arrangements available.
Main responsibilities include:
- Community and stakeholder engagement
- Assessment and restorative care coordination
- Providing ongoing support, advice, and guidance
- Ensuring quality care processes and risk management
About You
To succeed in this role, you will have:
- Current registration as a Registered Nurse or Allied Health professional
- Specialist certifications or qualifications in relevant disciplines are highly regarded
- Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
- A strong understanding of reablement, functional rehabilitation, and person-centred care models
- Proficiency in risk management, compliance, and quality assurance
- Excellent communication, problem-solving, and decision-making skills
- Competence in clinical documentation and care management systems
- Exceptional customer service, interpersonal, and liaison skills, with the ability to deliver person-centred services in community and home settings
- Experience managing a high workload and providing high-quality advice and support for diverse client needs in a fast-paced environment
- A good understanding of community care system and client needs
- Ability to build productive networks and relationships
- Initiative, problem-solving, and decision-making skills
- Experience working with operational KPIs and achieving business development targets
- Experience managing client budgets and handling time-sensitive tasks
- Driver's license and own vehicle
