Role Overview

This position is focused on enhancing the quality of life for individuals by providing high-quality support services.

About the Position

The incumbent will be responsible for coordinating restorative and rehabilitative care services, promoting independence and functional recovery.

Key Responsibilities:

  • Community engagement and stakeholder relations
  • Assessment and restorative care coordination
  • Support, advice, and guidance
  • Quality care processes and risk management

Requirements:

  • Current registration as a Registered Nurse or Allied Health professional
  • Specialist certifications in relevant disciplines (highly regarded)
  • Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
  • Strong understanding of reablement, functional rehabilitation, and person-centred care models
  • Proficiency in risk management, compliance, and quality assurance processes
  • High-level communication, problem-solving, and decision-making abilities
  • Competence in using clinical documentation and care management systems
  • Exceptional customer service and interpersonal skills
