Home and Community Physiotherapist Position

As a key member of our Home and Community Care team, you will have the opportunity to work with our aged customers in the North Metropolitan area of Perth. Our goal is to provide high-quality care programs that support their independence and well-being.


Job Responsibilities:
  • Conduct comprehensive assessments of each client's needs and develop personalized care plans that cater to their unique requirements.
  • Create engaging exercise programs that motivate clients to optimize their function and independence.
  • Through equipment assessment and prescription, identify solutions that enhance client autonomy and overall well-being.
  • Cultivate effective communication with clients, families, and caregivers to achieve successful outcomes.
  • Ensure seamless care coordination and accurate progress tracking through meticulous record-keeping.

Key Requirements:
  • Current registration with AHPRA as a Physiotherapist (essential).
  • Able to relate to elderly customers with empathy and work effectively with other healthcare professionals.
  • Excellent interpersonal and communication skills.
  • Proven ability to assess, plan, implement, and evaluate quality care.
