Job Description
\We are seeking a dedicated and compassionate Social Worker to provide high-quality care to patients and their families.
\The successful applicant will be responsible for providing counselling support, grief and loss support, advocacy, service provision, and complex discharge planning.

Required Skills and Qualifications
\To be successful in this role, you will need to have experience in counselling with a particular emphasis on adjustment to illness and hospitalisation. You will also need to have an understanding of the psychosocial issues associated with patients and their families and the support services available post-hospitalisation. Additionally, you will need to be knowledgeable about VCAT, TCP, and NDIS processes and be able to support discharge planning.
