We are seeking a skilled Aged Care Registered Nurse to deliver clinical expertise and leadership as a Clinical Nurse Consultant Grade 2.
Key Responsibilities- Implement comprehensive hospital avoidance strategies to optimize care for individuals aged 70 and over in Residential Aged Care Facilities (RACFs) and the community.
- Facilitate timely access to diagnostic, assessment, and treatment services to avoid hospital presentations or timely returns to residence with supportive strategies in place.
- Provide effective short-term case management for older adults while promoting evidence-based practice and advanced care planning within RACF staff.
- Lead and advise on nursing matters, formulating policy, guiding practice development, engaging in education, and research activities.
- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency.
- Minimum five years full-time postgraduate experience with three years full-time equivalent in the specialty field.
- Approved postgraduate qualification (minimum Grad Diploma/Certificate level) or working towards a Master's level qualification.
- Clinical leadership skills and ability to collaborate within a multidisciplinary framework.
- Effective communication and interpersonal skills across various mediums.
- Ability to undertake quality improvement and research activities influencing clinical practice.
