About this role

As a Transition Support Specialist, you will play a pivotal role in facilitating young people's successful transition to education, training, and employment opportunities. You will collaborate with colleagues to plan, deliver, and refer young people to these programs while providing essential support during their transition.

Key responsibilities include:

  • Demonstrating cultural capability and/or commitment to developing cultural competence to foster an inclusive environment aligned with human rights principles and supporting departmental initiatives that preserve cultural connections for Aboriginal and Torres Strait Islander peoples.
  • Preparing young people to participate in education, training, and employment programs as required and supporting the development of their interpersonal and self-regulation skills.
  • Providing effective support to young people within statutory and non-statutory frameworks during education, vocational training, work experience, and employment.
  • Maintaining accurate data and client records in accordance with relevant statutory, legislative, policy, and procedural requirements.
  • Establishing and maintaining strong working relationships with internal and external stakeholders and community partners.
