Role Overview

The Intensive Education Case Manager is a dynamic and challenging role that requires collaboration with a multi-disciplinary team to deliver intensive case management, transition support, and education initiatives for students at risk of disengagement.

This position will involve working closely with stakeholders, families, education providers, and community organisations to establish effective partnerships and implement best practice case management strategies.

Responsibilities
  • Provide intensive case management services to students who are at risk of disengagement, already disengaged or at risk of becoming involved with the youth justice system.
  • Develop and implement individual student plans and transition plans using an integrated approach that considers students' strengths and presenting concerns.
  • Establish and sustain effective partnerships with stakeholders, including families, education providers, and community organisations.
  • Implement culturally appropriate case management strategies to improve learning and engagement outcomes for all students.
Required Skills and Qualifications
  • Understanding of the impact of disability on individual functioning within an education context and evidence-based practices to support reasonable adjustments.
  • Experience working with First Nations communities, families and students and students from culturally diverse backgrounds.
  • Ability to work collaboratively in a multi-disciplinary team and establish effective partnerships with stakeholders.
