Job Description:

We are seeking an experienced and passionate professional to join our team as a Home Care Services Team Lead. In this leadership role, you will coordinate and oversee the delivery of high-quality, client-centered care, ensuring effective services.

Main Responsibilities:

  • Coordinate the Home Care Package Program, working in conjunction with senior management to ensure service delivery.
  • Develop and implement the case management model of care, monitoring client outcomes and service effectiveness.
  • Manage package expenditure to ensure services are provided within guidelines.
  • Contribute to marketing efforts to drive growth in Home Care Packages.
  • Identify and implement service improvements to enhance care quality and competitiveness.
  • Build and maintain relationships with key stakeholders, including government agencies, healthcare providers, and community organisations.
  • Ensure compliance with data collection requirements and funding targets.
  • Actively manage compliments, complaints, and feedback, striving for continuous improvement.
Required Skills and Qualifications:

To be successful in this role, you will need:

  • A minimum of 3 years' experience in aged care, home care, or a related discipline.
  • Sound knowledge of the Home Care and/or Aged Care industry, including the Aged Care Quality Code of Conduct and accreditation processes.
  • Strong communication skills with the ability to build rapport with internal and external stakeholders.
  • Experience in budgeting and fund management related to Home Care Packages.
  • Willingness to travel both intra and interstate.
  • Availability to participate in after-hours on-call duties.
