A permanent, full-time opportunity is available for a Service Team Leader to join our dynamic Home Care team. The successful candidate will be responsible for ensuring older Australians receive the right care at the right time.

This hands-on leadership role involves guiding a team of passionate Home Care Workers to deliver high-quality care that upholds dignity, independence, and quality of life.

About the Role

The Service Team Leader will assess client needs, conduct comprehensive assessments to understand physical, emotional, and social needs, and identify risks. They will develop, review, and update person-centered care plans aligned with client goals.

The role requires coordinating services, organizing in-home supports, and liaising with external providers to ensure timely, quality care. The successful candidate will also advocate and educate clients on their funding, choices, and rights.

  • Assess Client Needs: Conduct comprehensive assessments to understand client needs and identify risks.
