Job Role Summary

This full-time, permanent Case Manager position involves delivering tailored case management to vulnerable children and young people, supporting recovery from trauma and promoting positive life outcomes.

About the Job

We are seeking an experienced professional with a degree in Social Work, Psychology or related field who has experience with trauma-informed care and child protection. The successful candidate will have a strong understanding of the Intensive Therapeutic Care model and be able to develop and implement holistic case plans to support safety, wellbeing, and permanency.

Key Responsibilities:
  • Deliver comprehensive case management services to children and young people in our care
  • Support transitions through and out of care, ensuring a smooth transition for the young person
  • Maintain accurate documentation and reporting, meeting organisational requirements
  • BUILD STRONG RELATIONSHIPS WITH YOUNG PEOPLE, FAMILIES AND STAKEHOLDERS TO SUPPORT POSITIVE LIFE OUTCOMES
  • Coordinate referrals for after care and supported living services
Requirements:
  • Degree in Social Work, Psychology or related field
  • Experience with trauma-informed care and child protection
  • Understanding of the Intensive Therapeutic Care model
  • Unrestricted driver's license and own vehicle
