Role Overview

We are seeking a highly skilled and compassionate professional to fill this Care Partner position.

Key Responsibilities
  • Deliver personalized case management tailored to each client's goals and needs
  • Prepare and review care plans with clients to align with their objectives
  • Manage a caseload, conducting regular assessments and reviews
  • Build and maintain strong relationships with clients and families
  • Coordinate services across healthcare, nursing, and support teams
About You
  • You will thrive in this role if you have excellent problem-solving and advocacy skills, and a genuine passion for improving the lives of older individuals.
  • Previous experience in aged care case management (HCP/CHSP preferred)
  • A background in community or aged care service coordination is beneficial
  • Excellent planning, communication, and interpersonal skills are essential
  • You should be able to work independently while contributing to a multidisciplinary team
  • A valid driver's license and clean driving record are required
Requirements
  • Nursing qualifications (RN or EN) are advantageous
