About the Role

We are seeking a dedicated and experienced Service Delivery Manager to join our Community Care division. As a key member of our team, you will be responsible for the overall management and delivery of health programs and support services to clients in their own homes.

Your main responsibilities will include:

  • Coaching and supporting employees to foster a culture of employee engagement, dynamic communication, and service integration.
  • Coordinating and managing services to deliver a holistic approach to service delivery, focusing on the assessed needs and rights of clients.
  • Monitoring the evaluation of service provision through regular reviews and needs assessments involving clients, carers, and multidisciplinary teams.
  • Working with the Branch Manager to ensure client services are provided in an environment of continuous quality improvement, reablement-focused care, support, and innovation.
  • Supporting the Branch Manager's monthly review with the Business Operations Manager to ensure ongoing operational viability.

Requirements

To be successful in this role, you will need:

  • Tertiary qualifications and/or extensive management experience in a community service or health organization
  • A well-developed understanding of financial management
  • A strong understanding of the Community Care industry and the operational requirements of a Community Care Branch
  • Intermediate to advanced computer skills to use Ozcare's management and Community Care systems
  • A current Queensland Driver's License
  • A First aid and CPR certificate within one month of commencement
  • Proof of immunity or willingness to become vaccinated against Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
  • Vaccination against Influenza and Covid-19 is strongly recommended
  • A current NDIS Worker Screening Check and Blue Card (Working with Children Check)
  • A current National Police Check prior to commencement
  • Qualification as Key Personnel
