**Job Title: Community Support Specialist**

As a Community Support Specialist, you will provide support and assistance with daily living skills, connections with the community, personal and social development, employment, education, and training. You will also assist participants with activities of daily living (ADLs), money management, health and wellbeing, general life skills, and recreational activities as part of their care management plans.

**Key Responsibilities:**

  • Provide person-centered support and assistance with daily living skills

  • Support personal and social development through education and training opportunities
