Clinical Prevention Manager

We are seeking a skilled healthcare professional to fill the role of Clinical Specialist Preventable Chronic Conditions. The successful candidate will provide expert care and education to improve chronic condition management across remote communities in Northern Territory.
This is a challenging yet rewarding role that requires high-level clinical knowledge and skills in Preventable Chronic Conditions or related field. The ideal candidate will have strong decision-making and problem-solving skills, as well as a commitment to ongoing learning and professional development.
The Clinical Specialist will be responsible for providing sound professional advice, assessment, planning, review, evaluation, and coordination of comprehensive client care and casemanagement. This may involve traveling to remote communities by 4WD or light aircraft as required.
- Deliver specialist clinical outreach care to remote clients
- Provide support, advice, and education to primary healthcare staff on improving outcomes in early detection and management of preventable chronic conditions
- Accountable for assessment, planning, review, evaluation, and coordination of comprehensive client care and/or casemanagement
To be successful in this role, you will need:
- Current registration with Nursing and Midwifery Board of Australia or eligibility for same
- High-level clinical knowledge and skills in Preventable Chronic Conditions or related field
- Commitment to ongoing learning and professional development
- Ability to travel to remote communities by 4WD or light aircraft as required
