Supportive Psychosocial Services Coordinator

We are seeking a skilled and compassionate Social Worker to join our team in providing high-quality psychosocial support and advocacy to patients and families. The ideal candidate will possess strong clinical assessment and planning skills, as well as the ability to work collaboratively with multidisciplinary teams.
Responsibilities- Conduct consolidated psychosocial assessments and develop treatment plans that address the complex needs of patients and families.
- Work closely with healthcare professionals to provide evidence-based social work services and promote positive patient outcomes.
- Develop and maintain relationships with community resources and agencies to ensure seamless care transitions and support for patients and families.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practice in social work.
- Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the relevant professional body.
- Current Victorian Driver's Licence.
