As a key member of our multidisciplinary team, you will provide high-quality psychosocial support and advocacy to patients and families through assessment, counseling, information provision, access to community resources, and discharge planning.
You will work collaboratively with the broader healthcare team to develop and implement effective care plans, leveraging your consolidated psychosocial assessment and planning skills.
Your expertise in complex psychosocial assessments will be invaluable in ensuring we provide evidence-based social work services to all patients and families at our organization.
Required Skills and Qualifications- Bachelor of Social Work or Master of Social Work Qualifying, or equivalent as assessed and approved by the relevant professional association.
- A current driver's license for Victorian roads.
