About the PositionThe Homelessness Support Professional plays a vital part in assisting individuals to achieve housing stability, social inclusion, and self-mastery that enables long-term tenancy and improved quality of life.
Key Responsibilities- Deliver tailored case management services that align with industry models and best practices.
- Develop effective relationships with clients that promote self-determination and active participation.
- Conduct thorough assessments of needs, risks, and vulnerabilities.
- Collaboratively develop individualised case plans that include practical interventions and strategies to overcome barriers.
- Implement and monitor case plans in partnership with clients.
- Tertiary qualifications in Social Work, Community Services, Welfare, or a related field – degree qualification preferred.
- OR substantial sector knowledge and experience gained through professional roles and ongoing development.
- Demonstrated experience in client assessment, case management, and the social services sector.
- A current Working with Children Check (QLD Paid Blue Card).
- Current First Aid Certificate (if designated as a first aider).
- A valid QLD Driver's Licence.
