Our team is dedicated to providing support and care to individuals living in their communities, enabling them to maintain their independence and improve their quality of life.
As a Supported Living Facilitator, you will play a vital role in enhancing the daily lives of our clients. You will provide personalized support, help with daily tasks, and engage in activities that promote their physical and emotional well-being.
Your Role in Our TeamYou will work closely with clients and their loved ones to understand their unique needs and preferences. This may involve helping with medication management, meal preparation, and household chores, as well as accompanying clients on outings and engaging in recreational activities.
Every day will be different as you work on the road, visiting clients in their homes and providing one-on-one support. Your flexible approach will enable you to adapt to changing circumstances and ensure that each client receives the tailored support they require.
Key responsibilities include:
- Providing personal care and support to clients in their homes
- Assisting with daily tasks, such as bathing, dressing, and grooming
- Helping with meal preparation and household chores
- Accompanying clients on outings and engaging in recreational activities
