Geriatric Care Specialist

We are seeking a skilled and compassionate healthcare professional to join our team as a Geriatric Evaluation Management (GEM) Ward nurse. As a member of our multidisciplinary team, you will play a vital role in providing high-quality care to our patients.
Key Responsibilities- Provide comprehensive geriatric care to patients, including assessment, planning, and implementation of individualized care plans.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Develop and maintain strong relationships with patients, families, and caregivers to promote patient-centered care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Registered or Enrolled Nurse with experience in sub-acute care, dementia, and restorative care.
- Strong interest in and passion for caring for older people, complemented by acute care skills.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to work a 7-day roster and/or weekend work, with flexibility to adapt to changing patient needs.
