Job Title: Area Coordinator

Location: Shepparton VIC

Duration: Fixed Term, 22.8 hours per week

We are seeking a skilled and proactive professional to provide support and supervision for a team of case managers in our multi-disciplinary Hume community team.

The successful candidate will be responsible for ensuring clients receive the necessary support and care based on their funded level of package.

Key Responsibilities:

  • Promote a client-centric approach by recognising and promoting the rights of clients
  • Develop strong relationships with local services and networks
  • Ensure duty of care to both clients and staff is adhered to at all times
  • Provide support and supervision for a team of case managers
  • Maintain a small caseload of Home Care Package clients

Requirements:

  • Approved tertiary qualification in Social Work, Nursing, Welfare, Community Services or Allied Health
  • Demonstrated well-developed networking skills
  • An understanding of the aged care sector and community services
  • Current Driver's License
  • Experience in case management supervision and staff development
  • Sound understanding of the aged and community services legislative frameworks
