Job Description

The Transition and Post Care Support Officer is a critical role in supporting young people with complex support needs as they transition to adulthood.

The TPCS Officer works closely with Child Safety staff, government agencies, and non-government organisations to improve the quality of life for these young people, achieve stability in their living situation, and increase their involvement in education, employment, family, culture, and community.

Responsibilities
  • Provide specialist transition from care support that identifies and responds to the complex support needs of high-risk young people.
  • Promote and facilitate involvement of young people in goal setting and planning for their transition while supporting their connection to family, culture, and community.
  • Organise the provision of specialist services that address high risk and complex needs of young people and assist with the development of independent living skills.
  • Consult and develop relationships with government and non-government agencies to ensure efficient and effective service delivery.
  • Collaborate with regional line manager and senior program officer to ensure continuous quality improvement and alignment with regional and programmatic priorities.
  • Maintain sound, accountable, and legislatively compliant record keeping relevant to service delivery.
Requirements

The successful candidate will possess:

  • Relevant tertiary qualifications or experience in a related field.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Excellent organisational and time management skills.
  • Capacity to maintain confidentiality and handle sensitive information.
