As a Transition and Post Care Specialist, you will provide direct support to young people with complex needs aged 15-21 years who are at risk of homelessness during their transition to adulthood. Your focus will be on developing protective independent living skills, family networks and access to necessary specialist supports relevant to the individual's risks and needs.
Key Responsibilities:- Provide specialist support to high-risk young people who are at risk of homelessness, identifying and responding to their complex needs based on established frameworks for practice.
- Promote and facilitate the involvement of young people in goal setting and planning for their transition, while supporting their connection to family, culture and community.
- Organise the provision of specialist services that address the high-risk and complex needs of young people, assisting with the development of their independent living skills and enhancing capacity to maintain safe and stable housing.
- Consult and develop relationships with government and non-government agencies to ensure efficient, effective and financially supported service delivery.
- Collaborate with regional line managers and senior program officers within a hub and spoke model, ensuring continuous quality improvement and alignment with regional and programmatic priorities.
