Job Title: Community Care Manager

We are seeking a highly skilled Community Care Manager to join our team. This role will involve managing a caseload of elderly clients with Home Care Packages in the Sutherland Shire and Wollongong areas.

The successful candidate will have experience in community-based case management, preferably with government-funded in-home aged care services and budgets. They will also possess strong verbal and written communication skills, empathy and compassion, excellent problem-solving and critical thinking skills, good organisational and time management skills, cultural competence and ability to work with diverse populations, advocacy skills to meet clients' needs, and collaboration and teamwork skills to work effectively with interdisciplinary teams.

Key Responsibilities:

  • Regular communication with clinical scheduling and service delivery teams to ensure a smooth and comprehensive care experience for clients.
  • Assess clients' needs and develop personalised care plans.
  • Manage clients' funding budgets according to government guidelines to meet care requirements.
  • Onboard new clients and proactively manage existing ones with a compassionate person-centred approach.
  • Collaborate with clinical scheduling and service delivery teams to provide comprehensive care.
