Job Description
  • Coordinate and lead projects to enhance patient flow and care coordination across multiple service locations.
  • Monitor wait times and estimated discharge dates, identifying key barriers and implementing effective solutions.
  • Lead regular meetings with multidisciplinary teams to discuss patient movement, provide expert advice, and escalate critical issues as needed.

As a Clinical Nurse Specialist, you will collaborate with a dynamic team to deliver high-quality palliative care to community clients. You will take a leadership role in coordinating nursing and healthcare services, providing expert assessment and triage to optimize health outcomes.

Key Responsibilities
  • Provide evidence-based nursing care to patients of all ages and cultural backgrounds.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet unique needs.
  • Conduct thorough assessments and identify opportunities for improvement in patient satisfaction and efficiency.
  • Develop and maintain strong communication and interpersonal skills to work effectively with diverse stakeholders.
Requirements
  • Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency.
  • Relevant post-registration qualifications and at least three years' experience working in palliative care settings.
  • Demonstrated ability to work at a senior level within an interdisciplinary team, with a commitment to ongoing professional development and effective communication.
