Empowering Independence through Supportive LivingWe're dedicated to providing high-quality support services that empower individuals with disabilities to live fulfilling lives.About the RoleOur professionals work closely with participants to achieve their goals through community participation, independence, and supported independent living (SIL).Your Key Responsibilities
  • Strengthen participant abilities and build capacity to achieve goals.
  • Monitor progress and record effectiveness of supports.
  • Support participants to enhance and develop specific goals in their NDIS plan.
  • Provide direct support, supervision, and guidance to implement personal goals and NDIS plan.
  • Assist with appointments, follow-up activities, and plans.
  • Provide physical assistance and personal care as required.
  • Manage disability-related behavior or complex needs when necessary.
  • Maintain participant records and reports.
  • Support participants to use preferred methods of communication.
  • Interact and communicate with team members for consistent service provision.
Requirements
  • Certificate III in Individual Support, Community Services, or Social Sciences, or significant experience and skills in community services delivery.
  • Demonstrated knowledge and experience in delivering person-centered supports in SIL environments.
  • Ability to work in a team and independently.
  • Problem-solving skills and intermediate IT skills.
  • NDIS Worker Screening, Blue Card, or current Yellow Card with Positive Exemption Notice.
