**The Role**
Our Supported Accommodation Professionalsare responsible for supporting participants with their goals through community participation, independence & supported independent living (SIL).
**You will be responsible for**:
- Strengthening the abilities of participants and building their capacity to achieve their goals.
- Monitoring the progress of participant goals and recording the effectiveness of their supports.
- Supporting participants to enhance and develop specific goals as identified in their NDIS plan
- Providing direct support, supervision and guidance to participants to implement their personal goals and NDIS plan
- Assisting with appointments, follow up activities and plans.
- Providing physical assistance and personal care as required.
- Managing disability-related behaviour or complex needs when necessary.
- Maintaining participant records and reports.
- Supporting participants to use their preferred method of communication
- Interacting and communicating with other team members to ensure participants receive a consistent approach to service provision
- Ensuring that participants receive age and culturally appropriate person centred support
**Required Experience, Skills and Qualifications**
- Qualifications at Certificate III level or above in; Individual Support, Community Services, or Social Sciences, and/or significant demonstrated experience and skills in community services delivery is highly desirable or the ability to obtain Cert III within your first 6 months of employment.
- Demonstrated knowledge and experience in delivering "person centred" supports within a SILS environment.
- Ability to work in a team and independently
- Demonstrated ability to plan, prioritise and organise your workload in a way that contributes to high levels of successful participant outcomes.
- High level of problem solving skills.
- Intermediate IT skills.
- NDIS Worker Screening, Blue Card and or current Yellow Card - Positive Exemption Notice.
- Current QLD Driver's Licence
- **Please note that the role is supporting participants within a 24/7 home setting. The role requires you to be available to work, evenings, sleepovers and weekends.**_
