Our organization is seeking an experienced case management professional to join our team in regional Western Australia. This is an ongoing position requiring a flexible start date, offering competitive remuneration and benefits packages.
The Case Management Position:We are looking for a skilled case manager who has a passion for working with families and advocating for the best possible outcomes. You will be working closely with the community, social workers, and clinicians to provide support and guidance.
- Fully qualified Social Worker, Mental Health Nurse or Psychologist
- Experience within mental health is essential
- Able to obtain a National Police Clearance and Working with Children Check
We have vacancies available in the following locations:
Karratha, Kununurra (with housing support provided), Newman, Halls Creek, and Kalgoorlie
About the Role:This case management role involves working collaboratively with various stakeholders to provide holistic support services to clients. You will work closely with healthcare professionals to identify client needs and develop individualized plans to achieve positive outcomes.
You will also provide emotional support, practical assistance, and advocacy to ensure clients receive necessary resources and services.
This is a challenging yet rewarding role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
