We are seeking a highly skilled and dedicated Disability Support Professional to provide exceptional support to individuals with disabilities in their homes and communities.
Key Responsibilities:
- Establish strong relationships and collaborate effectively with staff, participants, family, and friends to achieve positive outcomes and ensure seamless service delivery throughout the participant's plan.
- Assist with liaison between participants' families and friends to explore new goals, ambitions, promote empowerment, and provide solid communication.
- Enhance participants' skills through active support and promote independence by developing their skills and personal development.
- Deliver high-quality and professional support in line with participants' plans, needs, and wants.
- Support participants in accessing their social community to engage with friends and social groups, promoting a healthier lifestyle.
- Implement strategies and programs to develop/enhance participant skills, identify areas of increased support, and opportunities for services to help achieve their goals.
Some participants may require assistance with medication prompts, while others may display complex behaviors. Experience and professionalism are highly recommended.
Essential Requirements- A current driver's license and a reliable vehicle.
- Prior experience in home care is preferred but not essential.
- Working with Children Check, First Aid Certification, and NDIS Screening Check are desirable but not required.
