Job Title: Clinical Care Coordinator

As a Clinical Care Coordinator, you will play a vital role in delivering high-quality patient care and contributing to the overall well-being of our residents.

  • Deliver clinical care to residents, including administering treatments as required and monitoring and evaluating care plans.
  • Manage and supervise care workers to ensure the highest standard of care is delivered.
  • Conduct comprehensive client assessments to identify individual needs and develop personalized care plans.

Key Qualifications:
  • Bachelor of Nursing degree and current AHPRA registration.
  • Expertise in managing and coordinating complex care plans.
  • Outstanding interpersonal and communication skills with the ability to work effectively with older Australians.
  • Experience working in an aged care environment.
