About the Role

As a Compassionate Care Manager - Elderly Services, you will be responsible for managing a caseload of community-based elderly individuals with Home Care Packages. Your primary goal will be to assess their needs, develop personalized care plans, and manage budgets in accordance with program guidelines to ensure each customer's unique requirements are met.

In this full-time permanent role, you will have the flexibility to manage your own schedule each week and work from home when you're not on the road visiting customers. While most of your caseload will be located in Hobart and the surrounding suburbs, occasional travel to Northern Tasmania may be required.

About You
  • You will have previous experience in community-based case management, preferably with Home Care Packages.
  • A current driver's licence and reliable vehicle with minimum Third Party insurance are essential.
  • Your strong communication skills, both verbal and written, will enable you to effectively interact with customers, interdisciplinary teams, and stakeholders.
  • Demonstrated empathy and compassion, along with excellent problem-solving abilities and critical thinking skills, will allow you to navigate complex situations and deliver exceptional results.
  • Cultural competence and the ability to work with diverse populations will be crucial in ensuring that our services meet the unique needs of all customers.
