We are seeking an experienced Clinical Care Coordinator to join our team at Derwent Views in Hobart, Tasmania. As a proud organisation since 1922, we foster communities of belonging and focus on giving back to older adults by providing dignified care and respect.

The successful candidate will play a crucial role in coordinating person-centered clinical care for residents, adhering to established policies, procedures, and professional standards. Working in collaboration with the Care Manager, the primary purpose is to concentrate on individual resident needs, ensuring the existence of a current and well-documented Care Plan that effectively addresses residents' needs.

About The Role:

The Clinical Care Coordinator works in collaboration with the Care Manager to ensure effective coordination of resident care, including:

  • Assisting with the admission of new residents to the facility, ensuring they are assessed.
  • Assessing the individual needs of residents and developing person-centered care plans in consultation with other healthcare professionals, ensuring care is tailored to meet their specific needs.
  • Collaborating with other healthcare professionals to deliver comprehensive care to residents.
  • Communicating effectively with residents' families and healthcare providers, keeping them informed about their health conditions, care plans, and any significant changes or concerns.
  • Maintaining accurate and up-to-date resident documentation, including care plans, behaviour charts, medication charts, observations, and/or incidents.
  • Providing and assisting with direct nursing care to residents, including activities of daily living (ADLs), administering and managing medication, catheterisation, conducting observations and monitoring, and wound management.
  • Actively participating in case conferences and relevant meetings.
  • Completing clinical assessments and reviews at required intervals and escalating as required, including COD status, daily wound care, and fall management.
  • Undertaking audits as required, including the Schedule 8 drug count.
  • Assisting with pharmacy ordering as required.
  • Checking alerts, overdue tasks, and progress notes to ensure they are completed effectively and to standard.
  • Coordinating infection control measures are in place and ongoing risk reduction strategies are being implemented.
  • Assisting with the investigation of clinical incidents and their close out as required.
  • Collating data and preparing information for reports as required.

Requirements:

The ideal candidate will possess:

  • A Bachelor of Nursing degree.
  • Current Australian Health Practitioner Regulation Agency (AHPRA) registration.
  • Nursing experience within a residential aged care facility, preferably at least 3 years of aged care experience.
  • Experience of working cooperatively with multi-disciplinary care teams.
  • Strong clinical assessment, documentation, and practice skills.
  • Effective communication and interpersonal skills.
  • Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
  • Experience in a similar clinical care role.
  • An understanding of the aged care accreditation process.
  • Working knowledge of quality improvement programs.
  • Current Driver's License.
