About the Role

We are seeking a highly skilled and experienced Senior Clinical Nurse to join our team at Programmed. As a Senior Clinical Nurse, you will play a key role in improving health outcomes and assisting clients to remain living at home by providing safe, responsive, and cost-efficient nursing care.

Key Responsibilities
  • Provide responsive, individualized services and support for each person and family
  • Utilize knowledge of clinical outcomes to initiate operational and clinical changes
  • Initiate and coordinate discharge and transfer to other services or programs both internal and external to Programmed
  • Network and liaise with clients, organizations, and the community
  • Ensure Programmed meets all clinical and healthcare regulatory standards and requirements
  • Identify and capitalize on new growth opportunities
  • Support our customers to achieve their goals
  • Accurate assessment and review of client care including; clinical suitability for staff delegation, managing health & issues and liaison with hospital discharge planners
Requirements
  • Minimum 3 years experience as a Registered Nurse
  • A current Registered Nursing qualification and sound clinical skills appropriate for complex care needs
  • Proven experience in a similar role within the community, aged, or disability care environment
  • High level of communication skills that display a commitment to care and empathy for clients and staff
  • Strong organizational, problem-solving, conflict resolution, and time management skills
  • Outstanding attention to detail
  • Ability to work in a high-pressure team environment to meet deadlines
  • Strong background in client relationship management
  • Use of Microsoft Office and an ability to learn new systems related to rostering and client management
  • Understanding of the direct care market
  • Background in business development and/or sales
