As a Community Registered Nurse, you will deliver high-quality clinical care that supports ageing well. From managing wounds and medications to educating clients and carers, your role is vital in helping people maintain their independence and dignity in familiar surroundings.

Key Responsibilities:
  • Health Education & Promotion: Provide valuable health education and support to enhance client and carer well-being.
  • Wellness for Independence: Deliver award-winning programs in-person and virtually to promote healthy living and independence.
  • Telehealth Services: Utilize Telehealth to monitor and improve client health outcomes.
  • Direct Care Services: Provide comprehensive clinical care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
  • Person-Centered Care: Assess, develop, and evaluate individual nursing care plans focusing on client goals and independence, supported by a multidisciplinary team.
Requirements:
  • Division One Registered Nurse with AHPRA, plus current Police Check, CPR, Blue Card, and Driver's Licence.
  • Minimum one year experience in community, acute, or aged care settings; advanced skills in areas like chronic disease, wound care, palliative care, and diabetes management.
  • Strong understanding of primary healthcare and consumer-directed care frameworks.
  • Confident in using electronic devices and client management systems.
  • Excellent organisation, attention to detail, and ability to work both independently and collaboratively.
