About the Role :

We are seeking a dedicated and compassionate Disability Support Manager to lead our team in providing exceptional care and support services to individuals with disabilities.

As a key member of our organization, you will be responsible for implementing personalized care plans, ensuring effective service delivery, and fostering a culture of excellence among our support staff.

Key Responsibilities :

  • Develop and implement individualized support plans tailored to each participant's unique needs and preferences.
  • Conduct comprehensive assessments of participants' needs related to their medical conditions and provide therapeutic interventions aimed at adaptation throughout the continuum of care.
  • Collaborate with multidisciplinary teams, including healthcare professionals and external partners, to ensure coordinated support services and optimal outcomes.
  • Maintain accurate and timely documentation of participant assessments, interventions, and outcomes, as well as prepare reports and summaries for case reviews and team meetings.
  • Leadership and Team Management :
    • Supervise and mentor a team of support staff to ensure the highest level of care and support to residents.
    • Provide guidance and support to ensure excellent customer service and maintain a positive work environment.

Qualifications :

  • Diploma in social work, human or community services, or a related field.
  • Proven experience in a similar role, with a minimum of 2 years in disability management.
  • Strong leadership and team management skills, with excellent communication, negotiation, and problem-solving abilities.
  • In-depth knowledge of relevant regulations, standards, and best practices.
  • Strong computer literacy, including proficiency in Microsoft Office.
