Right at Home Brisbane North And South, a leading provider of care services in the Aged Care and Disability Care Sector, is currently searching for an enthusiastic and dedicated Full-Time Care Coordinator to join our dynamic team. Responsibilities:
- Build strong relationships with clients and care workers, ensuring seamless communication and exceptional service.
- Manage a caseload of up to 35 clients living with various disabilities, coordinating their care plans, budget management, and promoting independence.
- Conduct home visits to introduce clients to our compassionate care workers and provide ongoing support.
- Utilise computer skills for efficient record-keeping and communication.

Requirements:
- Minimum of 12+ months of experience in the Aged and Disability Sector, preferably as a care worker.
- Valid QLD Driver's License.
- First Aid & CPR Certificate.
- Yellow Card/NDIS Worker Screening Clearance (Paid).
