Position: Care Coordinator
Location: Stones Corner Office, Brisbane

Your Role:

  • Relationship Building: Forge strong connections with clients and care workers, ensuring seamless communication and exceptional service.
  • Client Management: Oversee a caseload of up to 30 clients with various conditions, coordinating care plans, managing budgets, and promoting independence.
  • Home Visits: Introduce clients to our compassionate care workers and provide ongoing support through home visits.
  • Administrative Excellence: Utilise your computer skills for efficient record-keeping and communication.
  • Responsive Support: Answer calls from clients and care workers, ensuring their needs are met promptly.
  • Compliance: Conduct care plan reviews and maintain compliance documentation.

About You:

  • Experience: Minimum of 12+ months in the Aged and Disability Sector, or in a similar interpersonal role.
  • Qualifications: Valid QLD Driver's License, First Aid & CPR Certificate, COVID-19 Vaccinated, Yellow Card/NDIS Worker Screening Clearance (Paid), Federal Police Check.
  • Skills: Strong computer literacy and excellent interpersonal skills.
